from the book. The first update (October
2002) covered the period from manuscript
submission to publication. If you know of
work which you think should be included
here, or you disagree with what has been
included in some way, please email suggestions

alastair.macdonald@iop.kcl.ac.uk.
This update is cumulative- all new evidence
from the literature following manuscript
submission is included, and is still assigned
to the chapter headings from the book. However,
under each chapter heading the material
is divided into emerging themes and footnotes,
the latter mostly in the form of an annotated
bibliography.
Note that
• When new material relates to more
than one chapter, it may be repeated in
different chapter sections.
• No material specific to the topic
of delirium at the end of life is included
in this update; there is now a book on this
topic: Delirium - Acute Confusional States
in Palliative Medicine Caraceni and Grassi,
